"Misbehaviour" referred to risk-taking behaviour that puts people at risk of contracting HIV. In this case the doctor was referring to unsafe man-to-man sex in drama troupes (like zat pwe). Perhaps not the best choice of words but I suspect "misbehaviour" was used for censorship reasons.
When the doctor said "ethnic groups from the hilly regions", they were referring to the group known in Myanmar as Myo Thet Khami, a Rakhine subgroup from the Rakhine/Chin border area.
Your other question, "Why aren't more HIV programme workers members of other 'ethnic minority' communities?, you should direct to the NGOs.
