A further group of patients left behind by MSF France are those who tested positive for HIV but were still well enough with an adequate CD4 count at the time of diagnosis to not yet require ARV treatment. It is not clear how many people fall into this category.
This was brought to my attention by the case of a Burmese man known to us as Than who was treated for TB by MSF France in Mae Sot (TB number 5326) between February and September 2008. On 27/2/08 he had a CD4 count of 580 and was thus not given any ARV treatment. In January 2011 he came to our attention, extremely weak, and was sent to MaeSot Hospital where he was diagnosed and treated for Shigella enteritis and councelled but not started on ARV treatment. After discharge he was found some ARV therapy and seemed to stabilise but 3 weeks later failed to wake up for his medication on 11/2/2011 and died. "My good friend has just died" was the message that we received that morning notifying us of not just another unrecorded death but of another avoidable personal tragedy.
