Discharge letter
Dear colleague,
The present letter certifies that the following patient (please see detail below) has been discharged of our program from 24th of September 2009. He/she was followed for HIV/AIDS care services including ARV therapy provided by our projects based in Maesod. The project will be closed from the 1st of October following operational decision. The patient received 12 months of ARV drugs supply and prophylaxis if needed. The medical information attached to the present letter were valid for the period of time that the patient was under care of MSF project and are now referred as a summary, medical file and medical document to you to help further health management.
