What is a community?

An anonymous commenter asked the following three questions based on my question yesterday whether TOP Centres are 'community-run' ...

1. How do you define the word 'community'?
2. Do communities of men who have sex with men, transgendered people, or sex workers really exist in Myanmar?
3. Do people who are men who have sex with men, transgendered, or sex workers desire to be linked to such communities and receive services this way, or do they prefer to access general health services?

These are three good questions. How does one define a community when using the term 'community-run'? Allow me a few generalisations. Once a gay man, always a gay man. Once transgendered, always transgendered. Once an addict, always an addict. But once a sex worker, not always a sex worker? How many years after selling sex is a sex worker no longer a sex worker and no longer part of the community?
